donation requests

Azana Salon and Spa believes in giving back to the communities we serve through our Donations Program. Please review the eligibility requirements below to see if you qualify.

Eligibility:

  1. Qualify for a nonprofit status under the Internal Revenue Code.
  2. Benefit the local communities serviced by Azana Salon and Spa.
  3. Demonstrate sound and responsible financial policies and management.
  4. Satisfy Azana Salon and Spa that the project or program is not an unnecessary duplication.

Not Eligible:

  1. Funds for individuals.
  2. Projects of a political nature.
  3. Primary or secondary education.
  4. Beauty pageant participants.
  5. Religious organizations.
  6. Team sponsorship or fundraiser competitions.
  7. Event of program sponsorship.
  8. Scholarships.
  9. Class reunions.

Requests must be submitted in writing, including the following:

  1. A concise description, including purpose and objectives.
  2. Amount of request.
  3. Date of event.
  4. Contact person for procurement.
  5. Contact information, including mailing for procurement.
  6. Proof of nonprofit status under the Internal Revenue Code.
  7. The goal your event is trying to reach.

All requests will be evaluated by the Donations Department at Azana Salon and Spa. Any short-notice donations will not be accepted. Donations are made in the form of Azana Gift Cards to be used for products and/or services. Gift Cards can not be redeemed for cash or used for gratuity.
